(2) OBTAIN MEDICAL RECORDS FOR YOUR CONDITION (we can help track them down for you, if needed, by signing a medical release form, which allows our Doctor to obtain the records via fax or mail from other practitioners.)
(3) It is time to schedule your APPOINTMENT with our experienced Naturopath Physician, Dr. Jasmine May, NMD, to review your medical records and medical history. If you qualify, she will write a recommendation for medical cannabis.
(4) GET REGISTERED!Once the Doctor has made her recommendation(if you qualify), we will submit your application to ADHS(Arizona Department of Health Services)- the department that facilitates the medical Marijuana program. ADHS will mail your card once the application has been processed and approved.
